(1) If any person is vaccinated by a medical practitioner for the purpose of an International Certificate of Vaccination or Revaccination against cholera or smallpox, the medical practitioner shall complete and sign a certificate in a form provided by the Department of Health, and shall then stamp the form with a stamp which shall also be provided by the Department.
(2) The Director-General may, at his discretion, supply to such medical practitioners as he thinks fit forms and stamps for the purpose of enabling those medical practitioners to provide persons vaccinated by them with International Certificates of Vaccination or Revaccination against yellow fever.
