Returns must include—
(a) the name of the supplier; and
(b) the name of the person to whom the radio transmitter was supplied; and
(c) the address and telephone number of the person to whom the radio transmitter was supplied; and
(d) the date on which the radio transmitter was supplied; and
(e) the make and model of the radio transmitter that was supplied; and
(f) the serial number of the radio transmitter that was supplied; and
(g) the frequency or frequencies, or range or ranges of frequencies, fitted to the radio transmitter that was supplied.
