Preamble

At Wellington this 28th day of August 2006

Pursuant to section 21 of the Health and Safety in Employment Act 1992, His Excellency the Governor-General, acting on the advice and with the consent of the Executive Council, and on the recommendation of the Minister (as defined in section 2(1) of that Act), makes the following regulations.

1 Title

These regulations are the Health and Safety in Employment (Prescribed Matters) Amendment Regulations 2006.

2 Commencement

These regulations come into force on 10 October 2006.

3 Principal regulations amended
4 Schedule amended

(1)

Forms 3 and 4 of the Schedule are amended by omitting “[name and postal address of defendant]” and substituting in each case “[defendant’s full name or (if not a natural person) full legal name, and defendant’s postal address]”.

(2)

The section headed Details of service in form 4 of the Schedule is amended—

(a)

by omitting “[To be completed by enforcement authority on copy to be filed in Court only.]” and substituting “[To be provided for filing in Court.]”; and

(b)

by inserting “at [address**]” after “Served by personal service*/postal service*” in the second place where it appears; and

(c)

by inserting “** Full address at which the reminder notice was served.” below “*Delete which does not apply.”
