(1)
A person who is a medical practitioner, nurse practitioner, or specialist psychiatrist commits an offence if the medical practitioner, nurse practitioner, or specialist psychiatrist wilfully fails to comply with any requirement of this Act.
(2)
A person commits an offence if the person, without lawful excuse,—
completes or partially completes a prescribed an approved form for any other person without that other person’s consent; or
alters or destroys a completed or partially completed prescribed approved form without the consent of the person who completed or partially completed the form.
(3)
A person who commits an offence under this section is liable on conviction to either or both of the following:
imprisonment for a term not exceeding 3 months:
a fine not exceeding $10,000.
