Pursuant to section 88(1)(b) of the Births, Deaths, Marriages, and Relationships Registration Act 1995, His Excellency the Governor-General, acting on the advice and with the consent of the Executive Council, makes the following regulations.
These regulations are the Births, Deaths, Marriages, and Relationships Registration (Prescribed Information) Amendment Regulations 2015.
These regulations come into force on 3 July 2015.
These regulations amend the Births, Deaths, Marriages, and Relationships Registration (Prescribed Information) Regulations 1995 (the principal regulations).
After regulation 8A, insert:
An application for a name change under section 21A of the Act must contain,—
in relation to the person whose name is to be changed,—
full name at birth:
if the person has ever registered or deposited a name change in New Zealand or overseas, the person’s full name that resulted from the most recent registration or deposit:
date of birth:
place of birth:
contact details:
if the person whose name is to be changed is under 18 years old and is not, and has never been, in a marriage, a civil union, or a de facto relationship,—
full name and contact details of the guardian applying for the name change:
if the person whose name is to be changed is 16 or 17 years old, whether the person has consented to the name change.

This note is not part of the regulations, but is intended to indicate their general effect.
These regulations, which come into force on 3 July 2015, amend the Births, Deaths, Marriages, and Relationships Registration (Prescribed Information) Regulations 1995.
The amendment inserts a new regulation 8B that prescribes the information required by the standard form for an application for a name change.
